U.S. Army Awards Portion of $135M Medical Simulation Contract to ECS

Big news for US federal investment into healthcare simulation today with news that recently the U.S. Army Research Laboratory (ARL) has awarded two contracts to Engineering and Computer Simulations (ECS) for research & development services designed to demonstrate and advance state-of-the-art simulation and training technologies. The goal of the initiative, called Simulation, Training and Engineering Services (STES), is to produce training/simulation systems that can adapt over time to changing Army needs and conditions. STES addresses three technical areas — TA 1: advanced simulation; TA 2: medical simulation; and TA 3: squad and small-team research. ECS will be developing simulations for medical simulation and squad and small-team research.

Waymon Armstrong, CEO of ECS, said: “Our depth and range of simulation experience enables us to deliver effective simulations for both medical and warfighter training. Because our designers and engineers are highly skilled with decades of experience in all aspects of military executions, they quickly comprehend the need and design solutions that achieve the desired result. That means significant cost saving to the client, or if needed we can custom design the training tool.’

Simulation Modeling – From SimNET to the Internet of Simulations

From time to time we share stories from other industries that utilize simulation. There are plenty of lessons healthcare simulation can learn from the military, and today’s blog post provides a peak into the history and current status of simulation modeling. Here are some excerpts from Jose-Maria Lopez of Spanish operated Simware touching highlighting the history from “SimNET” to the Internet of Simulations. Such modeling tools enable programs to simulate future events and better plan for unforeseen scenarios. Imagine the applications for healthcare if we could simulate an entire hospital in real time and change just one wall placement at a time.

The Internet of Simulations Excerpt:

I read first about the origin of LVC simulation: the DARPA’s SIMNET program. The origin of LVC simulation for training as it is demanded now, based on high fidelity simulations, very inmersive 3D graphics and connected simulators in a network, was this SIMNET project. SIMNET was a DARPA sponsored and funded project and it is origin and importance for the readiness of the military forces is explained in the book : “The Pentagon’s Brain. An uncensored history of DARPA, …” by Annie Jacobsen. In her book, Ms Jacobsen explained how Capt (later Col) Thorpe, in 1978, got a very radical idea in that time : to link two flight simulators in order to allow a pilot and its wingman to train together in a common virtual world. Based on this first experiment made at Williams Air Force in Arizona, Capt Thorpe proposed the development of a network of flight simulators that would allow for ” real time dress rehearsals”. This idea was very radical in that time because the Internet’s predecessor, ARPANET, was only an small experiment in the late 70s. Capt Thorpe was able to go back to his idea in the early 80s when he was assigned to DARPA as a program manager.

In that moment he was aware of the classified work in ARPANET and was able to transform its idea in a new program in 1983 using the new technologies for networking developed at ARPANET; this project was named SIMNET and was focused on the main military priority in the early 80s: how to train and rehearse tanks battles as could be performed on Europe against the soviet armor forces. Now, 30 years later, SIMNET has evolved to LVC simulation and it is demanded  worldwide as a a basic tool to keep the operational readiness of the military and security forces. This reading has made me to compare how different has been the evolution of Arpanet and Simnet since they were created as DoD funded programs under the umbrella of DARPA:

  • Arpanet has evolved to Internet and has expanded its presence  everywhere, Internet has been able to offer an open platform to communicate people and machines located anywhere. Internet has been a big success, being able to position itself as the main platform to do any kind of business between companies and consumers (B2B, B2C, C2C, etc.) 
  • Meanwhile, SIMNET was the origin of many new technologies, not only for networking simulators but also to create impressive 3D virtual worlds. 3D has been able to go to the mainstream and now everyone has more 3D power in its smartphone than SIMNET had in its tank simulators in the 80s. But networking simulators is still a big issue even now in the 21th century, requesting large budgets and a lot of work performed by many skillful engineers, especially for high-fidelity simulators in the military or aerospace markets. The only market niche in which networked simulation has been able to go to the mainstream has been with the massive online multi-player games, but in this case each game is a propietary platform itself, closed to any external application.

Military Medical Simulation Program Highlighted on Local News

JOINT BASE LEWIS-MCCHORD, Wash. — Nothing compares to the real thing, but Army medic Jason Russell said a battlefield simulation at Joint Base Lewis-McChord comes close. “The animatronics, the bleeding, the smells,” said the staff sergeant, “Everything plays into that key that you don’t get stuck. You don’t freeze where you’re at.”

Part of the training at Madigan Army Medical Center’s Andersen Simulation Center involves sessions in a dark, smoky room filled with mannequin amputees covered in fake blood. “You can’t really train for this in a classroom,” said Russell.

Capt. Shannon Renfrow, a chief resident at Madigan, will have to go through the battlefield simulation room before she’s deployed overseas. As a doctor at Madigan she regularly participates in other medical simulations. Madigan was selected as a test site for a new birthing simulator. A rubber human belly and fetus replica allows doctors and nurses to practice C-section surgeries. The mannequin is called “C-Celia.” Like a flight simulator, it creates different complications for staff to handle.

While browsing products on the EMSWorld.com website, I came across a new trauma simulator I had never seen before — probably because it is brand new! The innovative folks at Innovative Tactical Technology Solutions (ITTS) have done it again with a new K9 Simulator to help police departments, military squads, or veterinary simulation champions to drill for K9 trauma simulations. I would also recommend EMS training programs consider investment in this learning tool. You may remember that I interviewed ITTS on camera a few years back to learn more about their TOM Manikin, which stands for Tactical Operations Medical Manikin — a product designed by TSgt. Keary Miller, a retired Air Force PJ (Pararescue man) who won the Silver Star for his bravery at the Battle of Takur Ghar in Afghanistan. USMC 3rd Battalion, 1st Marine Regiment, confront avatars, or virtual humans, while clearing a room at the Office of Naval Research Infantry Immersion Trainer (IIT) located at the I Marine Expeditionary Force Battle Simulation Center at Marine Corps Base Camp Pendleton. “For a long while a lot of the simulators out there provided a great capability but weren’t necessarily linked to training and readiness standards, and that’s where a lot of this effort is currently underway to look at that,” Adams said. To that end, the Marines are launching a massive effort at Camp Lejeune, N.C. this month to assess more than 20 warfighting areas to ensure the simulators available match the requirements laid out in the training and readiness (T&R) manuals.

Clemson virtual training program gets $3M in federal funding. Virtual reality work being undertaken by Clemson University is getting a boost from a $3 million National Science Foundation grant. Clemson’s Center for Aviation and Automotive Technological Education Using Virtual E-School, along with technical colleges around the state, will use the grant to advance the talent pipeline in aerospace, automotive and advanced manufacturing. Collaborators include S.C. Advanced Technological Education, Greenville Technical College, Florence-Darlington Technical College and Spartanburg Community College.

Combat Medic, a game-based system from Virtual Heroes, Inc., is a digital example of MedSim’s use in the U.S. military. This serious game from the Orlando-based division of Advanced Research Associates (ARA) helps prepare medical personnel to treat the top three causes of battlefield deaths: hemorrhage, airway management, and tension pneumothorax.

Steve Melito, a MedSim writer, recently interviewed Virtual Heroes Division Manager Randy Brown for an article in Combat & Casualty Care (C&CC) magazine. Brown was joined by Steve McIlwain, a senior producer. Excerpts from the interview appear in C&CC, but the full interview appears here on HealthySimulation.com.

1) What are some of the most important features and capabilities that you’d like readers to know about Combat Medic?

Combat Medic is the next step in serious gaming technology. We have effectively combined state-of-the-art game engine technology, medically accurate casualty conditions, and immersive environments, to create something truly special. To accommodate many training needs, we designed this application to deliver content through self-directed, team-based, and instructor-led training methods. Our overall goal was to prepare combat medics to treat the top three causes of preventable deaths on the modern battlefield: hemorrhage, airway management, and tension pneumothorax.

To maximize the learning benefit and scalability of the application, we integrated a robust difficulty system. This difficulty system introduces increasingly more challenging treatment scenarios. The system also increases the overall cognitive load of the participant by means of various distractions within the environment. The real-time, interactive immersion enables the cognitive load to resonate with the user. In theory, this translates to better handling of distractive elements encountered during real casualty treatment. Randomized variation of injuries aids in the neuroplasticity of the participant—they don’t just learn how to complete the training, they learn appropriate treatment methods.

A great feature of Combat Medic is its versatility. The application is highly customizable. For example, users may choose the contents and configuration of their aid bags; this stresses the importance of organization and preparation prior to casualty treatment, and it accommodates for varying equipment availability within the field. And Combat Medic is not only fantastic for individual training, but team training as well. During team exercises, users have the option to communicate using microphone-enabled headsets, which allows for effective team communication, triaging, and treatment coordination.

A critical component of any virtual training environment is performance assessment. We took a multi-pronged approach to assessment, to reach a larger demographic and to allow for different training environments. Our assessment system includes standard GO/NO GO performance criteria, casualty condition information, participant actions, and the ability to graphically view the casualty’s vitals throughout the scenario, as a function of elapsed scenario time. When a scenario is finished, participants and instructors can play the scenario back in full 3D with synchronized audio, to evaluate treatment performance and to identify areas for improvement.

Arguably the most important aspect of any training application is its accuracy. To achieve a true-to-life casualty physiological response during each scenario, we integrated our open-source BioGears® Physiology Engine. This validated engine accurately simulates the effects of participants’ actions on the casualty’s physiological condition. At the same time, it provides real-time casualty vitals with dynamic casualty behavior, based on the current physiological state.

Finally, a training application must be accessible. For Combat Medic to be easily available to medics around the world, it needs a relatively seamless method of deployment. For this, we use our proprietary Go Platform. Says Randy Brown, Virtual Heroes Division Manager, “the Go Platform is an exciting technology that combines simplified browser-based application delivery, virtual meeting spaces for mission briefing / debriefing discussions, interactive playback of training sessions, and modularity for future expansion.”
