Abstract:

A groundbreaking study done in the USA provided empirical evidence that substituting high-quality simulation-based experiences for up to 50% of traditional clinical hours was just as effective in providing learners with requisite knowledge and skills students would need upon graduation (Hayden, Keegan, Kardong-Edgren, & Smiley, 2014). In simulated based learning, learners work on complex problems in controlled environments that mimic real word scenarios (Lateef, 2010). Simulation-based education (SBE) is used in healthcare to facilitate students’ acquisition knowledge and skills through working with ‘ex-vivo’ patients in controlled safe learning environments without compromising the safety of real patients (Alinier & Oriot, 2022; Alinier & Platt, 2014; Aqel & Ahmad, 2014; Shin, Park & Kim, 2015).

Our project replaced 25% of clinical hours in medicine and surgery with simulations in a practical nursing program. A total of 22 students and their 3 instructors participated in this project. Evidence based approach was used throughout the design and implementation of the project to make sure that learning outcomes were met without compromising the quality of students’ learning. We documented the process of instructor training, curriculum development, implementation and feedback from students, staff and instructors. The project’s main purpose was to explore ways of addressing three important issues:

  1. Shortage of nurses and increase in enrollment,
  2. Scarcity of clinical placements and
  3. To address disparities of student experiences.

The modalities of simulation included medium to high-fidelity manikins (16%), standardized patients (SPs) (6%) and virtual reality (3%). EDI learning opportunities were woven into relevant scenarios. Students and instructor feedback shows that learners gained confidence, practiced skills they would not have encountered in traditional clinical, learnt the importance of critical reflective practice, collaboration, problem solving skills and resolving issues related to equity diversity and inclusion. The learnings from this project are being used to scale up the simulated based learning by replacing 50% of traditional clinical hours in continuing care for the internationally trained nurses that is currently running. The following academic year we will take these learnings and scale to over 400 students per semester at 25% replacement.

Our presentation will focus on the disruption of traditional clinical students’ experiences and the relearning process of new pedagogical strategies in nurse education. We will outline the pre-implementation activities, implementation process, the outcomes of the project and what we learned as discussion the way forward.

  2. Understand what steps were taken, logistics, and operational resources were used to execute replacing clinical hours with healthcare simulation.
  3. Apply these learnings from the presentation to the participant’s organization, to initiate or build on replacing clinical rotation hours with clinical simulation activities.

Dustin has been in the post-secondary education for over 16 years in a variety of roles including faculty, associate chair, senior manager of projects, academic program manager, and in the last 3 years as manager of the Olson Center for Health Simulation. He has led many projects to completion in varying sizes and enjoys operational leadership and strategic thinking. Dustin has a passion for experiential learning and educational innovation. He along with a small team, pioneered virtual reality training to NorQuest College over 7 years ago and today has incorporated multiple modalities of simulation at their sim center including virtual simulation games, VR, high and low fidelity mannikins, and standardized participant. NorQuest’s simulation center serves over 20 multidisciplinary health programs with simulation and has hosted over 3000 student simulation experiences last year and is estimating a growth of 30% for next academic year. Last year the NorQuest simulation center was able to achieve the goal of incorporating equity, diversity, and inclusion learning objectives in all scenarios as well as some antiracism scenarios to better prepare learners to be mindful and responsible practitioners. In his spare time, he enjoys running, hiking, camping, and spending time with his family.

Viola Manokore PhD
Faculty at Norquest College

 

Viola is an instructor and researcher at NorQuest College where she teaches in the department of Practical Nursing. She is a certified educator and holds a PhD in curriculum instruction and teacher education from Michigan State University. Her first and second degrees are in biological sciences. Viola is an internationally recognized scholar with university teaching experience at various levels. She was the recipient of the 2020 Gold Medal in leadership in teaching from Colleges and Institutes Canada (CICan). The CICan award recognizes a faculty member who has made personal growth and enhanced academic growth for students. In recognition of her research contributions at NorQuest College in 2019 Viola was the recipient of the “Quest for Excellence” research award. This award recognizes research contributions for faculty at NorQuest College. She was also the recipient of the 2020/2021 Scholarship of Teaching and Learning award from Alberta Colleges and Institutes Faculties Association (ACIFA). Viola has more than 25 publications in refereed journals. She has mentored and supervised a significant number of doctoral masters and undergraduate students. She is the founder and editor of a peer reviewed Journal of Practical Nurse Education and Practice. Viola is the lead on a project that is exploring mental health awareness and systems navigation in Alberta. The project has a newsletter that is widely distributed in Alberta and beyond.
