SimGHOSTS USA 2016 Day 2 Opens with Laerdal Sponsored Plenary Address From ASPE President

Today at SimGHOSTS 2016 USA Laerdal Medical sponsored the opening plenary address by Grace Gephart Director, PULSE Center at Arkansas Children’s Hospital and ASPE President.

About Grace

Grace Gephardt, MEd is the Director of the PULSE Simulation Center at Arkansas Children’s Hospital. She joined the PULSE Center in 2007 as their Standardized Patient Educator and became the Director in 2013. In her tenure, she has been involved in creating and implementing numerous comprehensive simulation scenarios for multidisciplinary use, including the development and launching of a national training in patient/family communications for pediatric chaplains. In 2014, she was nominated for and received the ‘Outstanding SP Educator of the Year’ award from the Association of Standardized Patient Educators and currently serves as President on the organization’s Board of Directors. Grace is also a member of SSH, SESAM and SimGHOSTS.

About ASPE

In 1963 a neurologist by the name of Howard Barrows discovered that a lay person could be trained to simulate illness and give feedback to medical students about their history and communication skills. He called this person a simulated patient and defined it as “a person who has been carefully coached to simulate an actual patient so accurately that the simulation cannot be detected by a skilled clinician. In performing the simulation, the SP presents the gestalt of the patient being simulated; not just the history, but the body language, the physical findings, and the emotional and personality characteristics as well.” Gradually, the use of the simulated patient began to grow in medical education. Educators found that simulated patients offered not only a variety of teaching opportunities for students, but also opportunities for testing student performance. Out of this testing environment grew the term “standardized patient” or “SP.”

As Gayle Gliva-McConvey explains, “An SP is a person trained to portray a patient scenario, or an actual patient using their own history and physical exam findings, for the instruction, assessment, or practice of communication and/or examining skills of a health care provider. In the health and medical sciences, SPs are used to provide a safe and supportive environment conducive for learning or for standardized assessments. SPs can serve as practice models, or participate in sophisticated assessment and feedback of learner’s abilities or services. The use of simulated scenarios involving humans is rapidly expanding to meet the needs of many high-risk service fields outside of human health care.” As simulated/standardized patient methodology grew, educators felt a need to develop an organization that could foster the growth of the profession that was creating and supporting this new methodology. Thus, in 2001, the Association of Standardized Patients was formed.

Since that time, our membership has grown along with the concept of standardized patients. Its use has expanded into many fields including dentistry, pharmacy, veterinary medicine, and allied health professions. Over the last decade, three simulation modalities have become intertwined – scenarios may now include SPs, task trainers and/or manikins; commonly known as “hybrid” simulations.

About “Dr. Voz”

Prior to his appointment as Vice President and Chief Medical Officer for Simulation for the Jump Trading Simulation & Education Center, “Dr. Voz” was the Director of Simulation Technology and Immersive Learning program for the Feinberg School of Medicine at Northwestern University where he provided central coordination and oversight for the undergraduate, graduate, interdisciplinary, and continuing medical education programs. In May of 2008, Dr. Vozenilek co-chaired the first Agency for Healthcare Research and Quality (AHRQ)- sponsored national consensus conference on using simulation research to define and develop clinical expertise. In his work at Northwestern he served as faculty for the Institute for Healthcare Research and its Center for Patient Safety, and continues to teach within its master’s degree program in health care quality and safety. He is currently the chair of the Simulation Academy within the Society for Academic Emergency Medicine and the Chair of the American Board of Medical Specialties Working Group on the use of Simulation for Maintenance of Board Certification for practicing physicians.

His use of simulation and research has included delivery of training and assessment of technical and non-technical competencies, and the use of simulation to emulate a clinical environment for work flow and process change, including electronic health care records (e.g. handoff and inter-hospital patient transfers). These projects demonstrate his expertise in the knowledge, skills, and attitudes required to create and sustain simulations for clinical environments and produce meaningful interventions with enhance health care reliability.

Day 1 Afternoon Plenary

Stephen Guinea is the Coordinator of Health Simulation for the Faculty of Health Sciences at Australian Catholic University (ACU). Within this role, Stephen has driven the strategic direction of health simulation at ACU focusing on simulation learning environments, staff capacity and capability and curriculum integration with the purpose of a quality student (and staff) experience of simulation. Stephen’s research interests include simulation design for authentic learning, clinical skills development and integrating theories of workplace learning into university curriculum. Stephen has received three learning and teaching awards including a 2013 Office for Learning and Teaching Citation for Outstanding Contribution to Student Learning. Stephen’s PhD research focused on designing sociocultural simulation for international nursing students as preparation for the clinical placement.

One of the greatest challenges facing health simulation programs is meeting the needs of the various stakeholders in a sustainable way. A significant part of this challenge is understanding the various needs and expectations, creating and communicating a vision, and enacting the vision in a way that is sustainable. The Faculty of Health Sciences at Australian Catholic University (ACU) has taken a strategic approach to the advancement of health simulation in terms of building facilities, capacity and capability of simulation technical services, education and training, curriculum integration and research. The success of such an approach is dependent on communication and relationships.

In this session, Stephen discussed that whilst very large student numbers, limited numbers of simulation technicians and an increasingly sessionalised academic workforce present particular challenges to advancing a simulation agenda for 13 health disciplines across 6 campuses, significant opportunities exist. The rapid advancement of simulation and simulation related technologies and a clear and consistent vision of simulation as a learning and teaching method shared by all stakeholders allows for thinking about how to “do” simulation in a way that meets these challenges. This session described the ongoing approach to building a sustainable simulation program at ACU and the essential contribution simulation technicians make to such success.

Day 2 Morning Plenary

This morning, Dr. Sprick gave us a deeper dive historical exploration of healthcare simulation, connecting the various industries that have contributed to our own.

Dr. Cyle Sprick is the Director of the Clinical Simulation Unit in the School of Medicine at Flinders University in South Australia. Dr Sprick has a PhD in Biomedical Engineering with a focus on clinical simulation. Clinically, Dr Sprick has been a practicing Paramedic since 1996, first in Texas, and then in Adelaide from 2000. Sitting at the nexus of clinical practice, education and biomedical engineering, Dr Sprick brings a unique perspective to clinical simulation. Before moving to Australia in 2000, Cyle worked for Oceaneering Space Systems (a NASA contractor) designing and building cryogenic (liquid air) based life support systems for underwater astronaut training, firefighters, and haz-mat workers. He also worked on the design of a liquid oxygen based life support system for the Mars mission space suit, and various robotic projects.

Augmented patients are an amalgamation of manikins, simulated patients (actors) and task trainers to provide the best of all three when simulating the deteriorating patient. Dr Cyle Sprick from Flinders University in Adelaide has been developing this style of simulation for over a decade. Dr Sprick discussed some of the work of the giants on who’s shoulders we stand as well as some of the new technological developments that are enabling this modality.

Who Should Attend SimGHOSTS Events?

Anyone responsible for the technical operation of a medical simulation lab including full-time or part-time Sim Techs, or clinical educators tasked with operating the day-to-day of simulation spaces. The role of the Simulation Technician varies with every simulation program. This talk will aim to provide practical tips for the new and mid-career level Simulation Technician. I will use practical examples from the earlier part of my career to the current to discuss how to work through sometimes difficult work flow dynamics and how the role of the Simulation Technician can be integral in bridging gaps to good simulation practices and operations.

Learning Objectives:

  1. Learn how to define your role when there isn’t much direction
  2. Learn to re-define your role within your organization through identifying operational gaps
  3. Learn the importance of good communication with your co-workers

About the Instructor:

Jeremiah Avarana, M.Engr, M.Ed. Jeremiah Avarana is currently a Simulation Technician at Tarrant County College’s (TCC) Department of Nursing supporting ADN- level simulation. He has worked as a Simulation Technician for 3.5 years total with experience in BSN and MSN simulation as well. Before simulation, Jeremiah completed advanced degrees in biomedical engineering and education with little clue of where it would one day take him. Healthcare simulation has answered that question. In his role, he has performed set-up and tear down of all nursing specialty simulations, programmed scenarios, taught faculty how to operate, set-up task trainers, acted in simulation productions, acted as standardized patient, and developed moulage for high-stakes and disaster simulation. His passion is to see the Simulation Technician role become a commonplace in every major simulation program and be a part of Simulation Technician training.
